I don't know about the first picture, but I think enemies going off the screen is considered a "feature" - I remember this same thing happening in Book I. It can be annoying if you're playing a melee fighter, but rangers and mages should be able to take them out at range.

I had the same problem with ghost images when using the tab key. It seems to only be noticeable in the dark.

Just ignore them.
