The game has crashed on me 4 times while moving on maps (different maps each time). The only thing I can think of that is common to all 4 crashes is that each time I was moving across the map with the mouse key held down. My machine is a Mac laptop running OS 10.6.8. There are two fixed locations for the book in Old Moonrise and Mirkland. You can sometimes by it in Moonrise and find it as a random treasure. The first time I found it, read it, and sold it before realizing it was part of the puzzle. Then I had to hunt for an old save game to figure out the clue. ...

Well, SpottedShroom thanks for that. It tells me what I needed to know: that the 3 keys that I need to map to adjust for handedness - ESC, Tab and Shift, cannot be mapped.
