Funny, when I first joined something similar happened to me.

I'm completely blanking on a few things.

1. The door combo on the first floor of Bastion Spire.

2. How to open the gate of the chest room on second floor (I think?) of Bastion Spire.

*3. Secondary entrance/exit to Dwarven mine (which from what I've read in this forum, may not actually exist yet ...

I agree. Book II > Book I > Book III.

I'd try emailing Basilisk Games, if this is the case. It is indeed many months overdue.

I've never understood why people were surprised that meeting with Karamiklan was the end of the game. Were none of you touching the quick travel markers?

For those items that increase their durability the higher in level you are when you find them. Not sure if there is already a list for these, so I might as well list the ones I've found in my time playing this game (if I've forgotten a few, please add on below).

I couldn't agree with you more. Book II remains the dominant game in most aspects. Book III is so short, in fact, that one guy speed ran the game in just over an hour (yeah it's a speed run, but still). I've tried XQuartz, X11, heck, I've even tried things along the lines of Wine. They either don't start, or they send all sorts of errors to Terminal. I haven't investigated very ...
