Do it the way many of the early CRPG did it (Including Ultima Exodus, I believe) And that is to say, make it so that lack of food doesn't HURT you, but just prevents you from regenerating health points throguh inactivity/rest.

I don't have much faith in FO3. Bethesda screwed up the Elder Scrolls series with Morrowind, then further diluted it with Oblivion. I don't expect them to stay true to FO hardcore roots. Instead, I expect a mainstream title with real time combat, no challenge, and a focus on graphics over gameplay ...
I had roughly the same experience as Saxon. I'm a huge fan of the first two (Still play them) but the 3rd was a bit messy. I play gothic 3 off and on now, but I don't care for the dumbed-down combat. (dumbed down when you compare it to the previous 2 gothics, anyway)
