1. I'm unsure what I'm doing wrong. I know I have to hit a lever in her lair to pass through but wherever I stand it says 'target obscured'. I'm playing a mage but have some throwing weapons on me.

2. Do you actually meet the witch? I've explored the lair but haven't come across her.

[+] SPOILER: In the corridor to the right there's a lever that you can't directly hit, but you should be able to hit the powder keg that sits next to it...
In the corridor to the left there's a window where you can shoot through.

Yes, you should be able to speak to the witch at some point.

Don't remember a powder keg there but will go back to the gate and have another go.

suggest NOT killing the witch with the lever/door method. Tree does not recognize the witch is dead.
