Something is wrong with my recipe...

When I open my recipe book and click on the Imbue ToHit recipe, it only places the mercury and auricflax into the slots, not adding the empty flask into its slot (I have 11 of them in my inventory). But this isn't the main problem. After manually adding an empty flask and attempting to mix, the entire thing explodes and I get damaged. I get a message: "be careful what you are mixing!"

I have tried adding all three items manually and mixing multiple times, reloading the game, and even temporarily raising my alchemy level to 30+ (through desperate means) to see if it was a skill problem, but I am still unable to make an Imbue ToHit potion. All other recipes work just fine, and both the ingredients and empty flask are automatically placed into their slots when I click on them (not that I'm lazy, but this is what is suggesting to me that something may be bugged).

Am I completely missing something? Or is my save game bugged?

You don't imbue potions with ToHit. You imbue weapons. The recipe puts the proper ingredients in your mixer; you need to choose a weapon to place in the slot.
