Some ideas ..

Artifacts which add to stats or skills if you keep them in inventory (make inventory mgmt more interesting, and get over the 'not enough skillpoints for rogues' type complaints).
Timed quests (or at least rewards which go down if you don't do the quest fast enough).

Some indications of game time (month, year, whatever) not just wallclock time.
More trainers (dodge, thieving skills)
More in-game dialogue hints (like 'you know you can steal with impunity while you are invisible, don't you' from the thief trainer, or 'watch out for the damn ghosts, coz they can curse you' from the crypt folks. or 'You need to get that disease seen to, because it won't wear off on its own', from anyone who notices you are diseased).
